Almost everything else on this site exists because prevention did not happen, or happened too late. That is not a moral point — decay and gum disease are painless until they are advanced, which is precisely why they get missed. Preventive dentistry is a system for finding problems while they are still small and cheap.
Exams and x-rays
An examination checks the things you cannot see: decay between teeth and under existing restorations, changes in the gums, and soft-tissue changes that warrant a closer look. X-rays are the part that finds problems early, because the surfaces where adult decay most often starts are the ones hidden between teeth.
Digital radiography, which is what we use, delivers a substantially lower radiation dose than the film it replaced and produces images immediately, so anything found can be discussed while you are still in the chair.
Cleanings
A professional cleaning removes plaque and hardened tartar from the tooth surfaces at and just below the gum line. Tartar cannot be brushed off once it has formed — that is the whole reason the appointment exists.
The usual interval is every six months, but it is a starting point rather than a rule. Patients with a history of gum disease are typically seen every three to four months, because the interval that keeps a healthy mouth healthy is not the interval that keeps a treated one stable.
Sealants
Sealants are thin coatings applied to the chewing surfaces of back teeth, where the natural grooves are too narrow for a toothbrush bristle to clean properly. Application is quick and completely painless — the tooth is cleaned, the sealant applied, and that is the appointment.
They work. A Cochrane review found resin sealants cut decay in back teeth by 11% to 51% over two years compared with no sealant. They last roughly five to ten years depending on diet and habits, and they are checked at routine visits because a chipped sealant needs topping up rather than replacing.
They are most commonly recommended for children and teenagers, whose newly erupted permanent molars are at the highest risk, but adults with deep grooves and no decay in them are candidates too.
Fluoride
Fluoride works by remineralising enamel — rebuilding the mineral that acid has begun to strip — and making the surface more resistant to the next acid attack. It is among the most thoroughly studied measures in dentistry, and it is the reason toothpaste is worth using rather than just the brush.
What you do at home matters more
Two dental visits a year is roughly two hours. The other 8,758 are yours, and they decide most of the outcome.
- Brush twice daily for two minutes with a soft brush at about 45 degrees to the gum line, and replace it every three to four months, or sooner once the bristles splay.
- Clean between the teeth daily. A toothbrush cannot reach those surfaces, and a Cochrane review found interdental cleaning in addition to brushing reduces gum inflammation more than brushing alone. Floss, interdental brushes and water flossers all count.
- Watch frequency, not just quantity. How often sugar reaches your teeth matters more than how much arrives at once, because each exposure restarts the acid cycle.
When to come in sooner
Do not wait for the next scheduled visit if you notice bleeding when you brush, gums that are receding, a tooth that has become sensitive to hot or cold, a persistent bad taste, or any change in how your teeth meet when you bite. All of those are early signals, and early is the entire point.
